Family forms
The safety and consent forms every ACA family fills in once.
All five forms are combined into one short online packet you fill in once; after that we only ask each year whether anything changed. Log in to your family account to get started.

Names the adults, 18 or older, who are allowed to pick up your child from school.

Keeping everyone safe
Pickup, safety, and how we stay in touch.
Children are released only to their parents or guardians and to adults you have authorized for pickup. Pickup adults must be at least 18 years old and may be asked to show a photo ID.
Need to pick up your child before dismissal? Please come to the office and sign your child out — don’t go to the classroom. A parent or guardian, or an adult you’ve authorized for pickup, may sign a child out; please bring a photo ID.
If your child needs medication at school, please provide it yourself, clearly labeled with your child’s name and instructions.
Keep your child’s allergy and health information current, and let the office know right away when anything changes.
You decide whether your child appears in school photos and ACA publicity. You can change these choices anytime in the Family Portal.
Teachers and office staff often message parents on WhatsApp, one-to-one and in class or school groups, where other members can see your number. Please make sure the cell number on your family’s record is the one you use for WhatsApp.
